Member/Guest Agreement Forms

All Guests and Members are required to complete this Agreement, including class attendees. Our experience has determined that it is better to provide the same Agreement to each individual visiting Maker Works. This reduces misunderstandings of our rules and requirements for all persons in the Maker Works facility.

You may print out and read this Agreement in its entirety, initial the bottom of each page, and sign where required. Bring this document with you to your first class or member visit or you may fill one out when you come in, but please allow 10 minutes to do so before the start of your class.

Please note: If the person taking a class or becoming a member is under 18, the forms should be completed with their information but signed by a parent or guardian. 

The Agreement includes:

• Section 1: Personal Information – your contact information (leave Member Number blank)

• Section 2: Release, Indemnity and Assumption of Risk – a legal document, please read thoroughly (If the attendee is a minor, a parent or guardian must complete this and enter the minor’s name in part 6.)
